WAIVER
Please download and fill out the application & waiver and then submit them via email, along with a copy of your vaccine records and I will respond to you shortly to schedule your assessment or trial day for your dog.
Please fill out these two documents with as much detail you can share with us. On the waiver you can simply type your initials on point #13.
Also do remember to insert your name, date and the dogs name on that waiver. Please let us know what weekly schedule you need.
