Dog Daycare & Boarding Waiver

*I hereby represent that I am the legal owner of the dog described above as (the “Dog”) to be enrolled in dog daycare with a Membership, and will follow the rules and policies for that membership.

*I hereby release The PupHaus, its owners, representatives and agents from all liability which I, the dog, any third party or their pet may suffer, including specifically, but not without limitation, and injury or damage whatsoever arising from my actions or failure to act, the Dogs actions or generally the Dog’s attendance and participation in the daycare and your use of the website under the Umbrella of The PupHaus.

*I hereby agree to indemnify and hold harmless The PupHaus, its owners and agents from any and all claims, or claims by myself or third party accompanying me to a function of the Doggy Daycare, or while attending the premises thereof, as a result of any action or failure to act by me or the Dog.

*I hereby represent that the Dog is of good health and has not been ill with any known contagious diseases within the last thirty days. I recognize that the health of the Dog is my personal responsibility. I hereby represent that all required vaccinations for the Dog are up to date and will continue to remain so for as long as the Dog attends Doggy Daycare. I will also promptly provide proof of all booster vaccinations and stay up to date.

*I further understand and agree that in joining the daycare, The PupHaus has relied on my representation that the Dog is in good health and has not harmed or shown aggression or threatening behaviour towards any person or any other dog.

*I understand that as a member of The PupHaus daycare I will attend regularly, as per my package, each week, or use my flex days within its time frame, to maintain my membership, or my days can be lost. I understand that as a PM, I need to notify the daycare if I can't attend due to unforeseen circumstances, or my day will be lost.

*I understand that as a member of The PupHaus dog daycare, I own my spot and am responsible for it up until and including the time I give two weeks notice to cancel my membership. Failure to uphold this policy will be followed up with collections and will incur late fees up until paid in full. This included any new invoices issued before notice is given to cancel.

*I further understand and agree that in joining the daycare, The PupHaus has relied on my representation that the Dog is in good health and has not harmed or shown aggression or threatening behaviour towards any person or any other dog.

*I understand that by bringing the Dog to an open facility means that they will be interacting with other dogs. Minor cuts or scratches are inevitable, and, though the dogs are carefully supervised at all times, and the owners do their utmost to keep them safe, there is no warranty or representation by The PupHaus that a more serious incident will not occur.

*I further understand and agree that The PupHaus and their handlers will not be liable for any problems that might develop with the Dog, including, but not limited to, sickness, disease, injury, running away and death, provided that reasonable care and precautions are followed.

*I understand I am solely responsible for any harm caused by the Dog to another dog or the handler while they are with The PupHaus. If for any reason, The PupHaus is unable to continue caring for the Dog, I must remove the dog from daycare or cancel any further sessions with the dog.

*I further understand and agree that any problem that develops with the dog will be treated as deemed best by the owner of The PupHaus, at their sole discretion and that I assume full financial responsibility for any and all expenses involved. If the dog is removed from the daycare, they do not offer any refund.

*I understand that The PupHaus is insured, and if injury, loss or death occurs, we are covered under the daycare insurance, if we are members of the daycare at the time of incident.

*The hours of operation for The PupHaus are listed on the website and updated at all times to reflect holidays and open times. I agree that the management of, retains the right to amend the hours of operation at any time.

*I understand that I must pay in full for services rendered before picking up the dog from daycare. The PupHaus reserves the right to permanently remove a Dog from its daycare at any time.

*I understand that I must walk the Dog in and out of the facility on a leash, and in a safe and expedient manner. If I chose to go off leash, it is done at my own risk and responsibility.

*I hereby give my consent to use all photographs taken of my dog according to the Terms & Conditions.

*The PupHaus shall be the exclusive owner to the result and all proceeds of such taping, photography and recordings.

*If the Dog attends on multiple dates, then this Agreement applies to each and every date, until the last day attending. 

*All the above points are included under this agreement. Failure to follow our rules and policies can also result in loss of service and with no chance of a refund. 

*I, the Owner, on the date listed below, certify that I am eighteen years of age or older and have the legal capacity to enter into a binding contract.

*I am the only owner of the Dog, or if I am not, then I have advised The PupHaus dog daycare of who the other owner(s) is (are).

*Further I certify that I have read and understand the terms and conditions set forth in this agreement.

*I agree and accept all the terms, conditions, and statements of this agreement.
