*What is your dog training history? What is your dogs training level?

*How do you respond to your dog doing something wrong in the home?

*How does your dog react to strangers or other dogs out in public?

*Does your dog fear or dislike any types of people, or types of dogs?

*Is your dog frightened by any noises or objects?

*Is your dog possessive of toys, food or objects?

*Does your dog have any known behavioral triggers?

(e.g., resource guarding toys/food, barrier barking, fear of loud noises, separation anxiety)

*Has your dog ever bitten another animal or human, or been involved in a serious dog fight?

(Please answer honestly; this ensures the absolute safety of our closed pack).

*List the following if any apply to your dog: hip dysplasia, arthritis, loose kneecaps

This is required, as an outbreak of fleas would shut the daycare down.

*Is your dog fully up-to-date on all core vaccinations (DHPP, Rabies, and Bordetella)?

*Check if your dog is vaccinated for:

Bordatellosis (kennel cough)

This is not mandatory, but recommended, even though it only protects them from one of the many strains going around, so they could still get sick.

Its a flu with a lot of coughing at night.

*Check if your dog is vaccinated for:

Leptospirosis

This is not a mandatory shot, but recommended due to the fact that we have racoons and other wild life in the area.

*Can we give your dog treats while in daycare?

*Where are your dogs favorite petting spots?

*Is your dog sensitive about any parts of their body?

*Please list any comments about your dog that your feel will be helpful to us i.e. digging, whining, barking or potty training problems.

Not disclosing behavioural problems with your dog can cause cancelation of membership, and no refund, so be honest and I will work with you to solve it.
