Registration and Waiver

Please fill out a registration for Daycare or for Bed & Buddies boarding and submit along with vet records.

Our waiver is required for both services. Please check all points on the waiver to confirm you have read it, and don't forget to sign it before submitting. I need both documents completed, with the vaccine copy attached to move ahead with a booking.

Upon receipt, I will contact you to schedule your dogs trial day, if I have suggest you will need one. We will otherwise book your first day in. It must be on a weekday while most of the pack is here.

I hereby represent that I am the legal owner of the dog described above as (the “Dog”) to be enrolled in Doggy Doolil daycare

I hereby release Doggy Doolil Daycare, its owners, representatives and agents from all liability which I, the dog, any third party or their pet may suffer, including specifically, but not without limitation, and injury or damage whatsoever arising from my actions or failure to act, the Dogs actions or generally the Dog’s attendance and participation at Doggy Doolil Daycare or their Website

I hereby agree to indemnify and hold harmless Doggy Doolil Daycare, its owners and agents from any and all claims, or claims by myself or third party accompanying me to a function of Doggy Doolil Daycare, or while attending the premises thereof, as a result of any action or failure to act by me or the Dog

I hereby represent that the Dog is of good health and has not been ill with any known contagious diseases within the last thirty days

I recognize that the health of the Dog is my personal responsibility. I hereby represent that all required vaccinations for the Dog are up to date and will continue to remain so for as long as the Dog attends Doggy Doolil Daycare. I will also promptly provide proof of all booster vaccinations and stay up to date

I further understand and agree that in joining the daycare, Doggy Doolil Daycare has relied on my representation that the Dog is in good health and has not harmed or shown aggression or threatening behaviour towards any person or any other dog

I understand that by bringing the Dog to an open facility means that they will be interacting with other dogs. Minor cuts or scratches are inevitable, and, though the dogs are carefully supervised at all times, and the owners do their utmost to keep them safe, there is no warranty or representation by Doggy Doolil Daycare that a more serious incident will not occur

I further understand and agree that Doggy Doolil Daycare and their handlers will not be liable for any problems that might develop with the Dog, including, but not limited to, sickness, disease, injury, running away and death, provided that reasonable care and precautions are followed

I understand I am solely responsible for any harm caused by the Dog to another dog or the handler while they are with Doggy Doolil Daycare. If for any reason, Doggy Doolil Daycare is unable to continue caring for the Dog, I must remove the dog from daycare or cancel any further sessions with the dog

I further understand and agree that any problem that develops with the dog will be treated as deemed best by the owner of Doggy Doolil Daycare at their sole discretion and that I assume full financial responsibility for any and all expenses involved. If the dog is removed from the daycare, they do not offer any refunds

The hours of operation for Doggy Doolil daycare are listed on the website and updated at all times to reflect holidays and open times. I agree that the management of, retains the right to amend the hours of operation at any time

I understand that I must walk the Dog in and out of the facility on a leash, and in a safe and expedient manner. If I chose to go off leash, it is done at my own risk and responsibility

I understand that I must pay in full for services rendered before picking up the dog from daycare

Doggy Doolil Daycare reserves the right to permanently remove a Dog from its daycare at any time

I agree that the Dog may be videotaped, photographed, and /or recorded. Doggy Doolil Daycare shall be the exclusive owner to the result and all proceeds of such taping, photography and recordings

If the Dog attends Doggy Doolil Daycare on multiple dates, then this Agreement applies to each and every date, until the last day attending

These above points apply to all Bed n’ Buddies and board and train clients as well

All the above points are included under this agreement. Failure to follow our policies can also result in loss of service and with no chance of a refund

I, the Owner, on the date listed below, certify that I am eighteen years of age or older and have the legal capacity to enter into a binding contract. I am the only owner of the Dog, or if I am not, then I have advised Doggy Doolil daycare of who the other owner(s) is (are). Further I certify that I have read and understand the terms and conditions set forth in this agreement
