To fill out this applicaton you need to turn on the 'insert' by clicking the key on your keyboard, in order to keep the text from moving around. You will find your 'insert' key at the top right hand corner of your keyboard.

The waiver works the same as the application. There is also a question on it that requires an initial, and you can type in your initials on that line. Also remember to insert your name, date and the dogs name on that waiver.
